Membership fees for 2008:                                                 

$200.00 Early Registration  10/21/07-01/14/08                                

$225.00 Open Registration            01/15/08-04/30/08

$250.00 Late Registration             05/01/08-7/15/08

Football fee includes: RENTAL of full game uniform including all pads, equipment bag, league, game &practice field fees.

Not included: Molded rubber football cleats and mouthpiece, girdle, practice pants

I give approval for participation in all activities of Salinas Pop Warner, Inc. during the 2008 season. I assume all risk and hazards incidental to the program. In case of emergency, I hereby authorize any necessary treatment.

YOU WILL NEED TO SUBMIT THE FOLLOWING BY

June 31, 2008:

o           Completed Pop Warner Player Contract & parent consent form

o           Completed Pop Warner medical Form dated January 1st 2008 or after stating the athlete may participate in the 2008 Pop Warner season.

o            Completed Parent Pledge form.

o            Copies of report card (or equivalent) for ALL academic grading periods during the 2007-2008 school year. (must maintain 2.0 (“C”) or better average in order to participate.) THIS IS DUE NO LATER THAN 7/1/08

Players must be no more than 6 pounds

above their weight category on Aug. 1, 2008 and will be cut due to weight restrictions if space is unavailable on the next division or the player  does not qualify for the age group minimum.

 

THE FOLLOWING MUST BE RECEIVED BY THE END OF OPEN REGISTRATION:

o         A current 2x2 or 2x3 photograph. No hats or dark glasses. No polaroids

o         COPY of birth certificate or

o         Passport. (Those who played last year do not need to supply this document.)

o         Completed & signed Volunteer form or $300.00 buyout check if you choose not to volunteer. This is a volunteer operated organization and one parent will receive a schedule prior to the PPKO.

o         Your registration check made out to Salinas Pop Warner, Inc.
